If you have had contact with the University Ombuds Office as a visitor or as a resouce person, thank you for taking a few minutes to fill out this survey. Please complete Section I, if you were a visitor to the Office, or Section II, if you served as a resource person. Persons who have had occasion to experience the services of the Ombuds Office in both roles should complete both sections of the survey. Section III (General Information) is designed to be completed by all respondents.

Your confidential feedback assists us in continuing to serve as a safe, confidential, neutral, and independent resource for the entire University community.

ANONYMITY: The survey data is sent electronically to the Office of Institutional Research and Planning (OIRP), where it is downloaded and sent directly to the Office of the University Ombuds for analysis.
