Survey for Users of the University Ombuds Office
If you have had contact with the University Ombuds Office as a visitor or as a resource person, please take a few minutes to fill out this survey. Please complete Section II if you were a visitor to the Office or Section III if you served as a resource person. Persons who have had occasion to experience the services of the University Ombuds Office in both roles should complete both sections of the survey. Sections IV, V, and VI are designed to be completed by all respondents.

DEFINITIONS:
You are a "visitor" to the University Ombuds Office if you contacted the office about a personal concern or complaint. You are a "resource person" if you contacted the University Ombuds Office for information, policy consultation, or other type of assistance or an Ombuds contacted you for information or assistance.

Your feedback assists us in continuing to serve as a safe, confidential, neutral, and independent resource for the entire University community. Your name will not be associated in any way with the survey findings. Only summary group data will be analyzed and reported. Individual responses will not be identified.
